Community Service In Arkansas
The providers and staff of East Arkansas Family Health Center (EAFHC) are committed to the communities we serve in Crittenden, Poinsett, Mississippi, and Phillips counties. Our goal is to help residents in these counties obtain affordable health care services and prescription medication.
Some of the most important community outreach services we provide include:
- Medicaid enrollment assistance through the Arkansas Works Program
- Health marketplace insurance enrollment assistance through the Affordable Care Act
- Help obtaining affordable prescription medications
- Free health screenings at our annual health fair
- Transportation to and from our offices
Medicaid Assistance
At EAFHC, our caring and compassionate outreach support staff can provide comprehensive Medicaid assistance for you and your family, from understanding your eligibility to completing and filing the paperwork.
Prescription Assistance
We can also help you receive prescription medications that you and your family members need at prices you can afford. If you do not have pharmacy benefits or you are uninsured or underinsured, you may be eligible to receive prescription medication through EAFHC’s Medication Assistance Program (MAP).
Our on-site pharmacies, available at our West Memphis and Lepanto locations, partner with many pharmaceutical companies to offer discounted prescription medications for the residents of eastern Arkansas.
To qualify for the Medication Assistance Program, you must meet the following criteria:
- Be a U.S. citizen or a legal resident
- Be an established patient of EAFHC
- Meet household income guidelines
- Have no prescription insurance
Annual Health Fairs
EAFHC also reaches out to our community through our annual health fairs held throughout the year. At our health fairs, we offer free screenings for vision, hearing, oral health, diabetes, cholesterol, and high blood pressure (hypertension).
Transportation
In addition, EAFHC provides transportation to and from most of our locations, as well as for patients who need additional services not performed at the clinic, such as mammograms.
To speak to someone regarding these services, please call the office you normally visit or ask a staff member at your next appointment. We will be more than happy to assist you.
