East Arkansas Family Health Center (EAFHC) follows the Patient-Centered Medical Home (PCMH) approach, which means we strive to have you see the same provider every time you visit. Not only does this help you and your provider develop a relationship, but it helps your provider deliver the most personalized, coordinated health care possible.
Our highly capable doctors, physician assistants, and nurse practitioners offer comprehensive care for a wide range of acute injuries, illnesses, and conditions, as well as help you manage chronic health conditions. For your convenience, we also provide the following services on-site:
- Lab draws and X-rays
- Well-child visits, immunizations, and EPSDT screenings
- Women’s health services (OB/GYN care and family planning)
- Men’s health care
- Behavioral and mental health care
- Pharmacy at West Memphis and Lepanto locations
- Ophthalmology services

- Nutritional counseling (diabetes education and healthier eating tips)
At EAFHC, we treat patients of all ages from Crittenden, Poinsett, Mississippi, and Phillips counties. Our providers will deliver the care you need when you need it. They will explain diseases, treatments, and test results in an easily understandable manner, and help you make decisions about your care.
No matter what stage of life you are in, the doctors, physician assistants, and nurse practitioners at EAFHC offer timely and affordable services to address all of your health care needs. We accept Medicare, Medicaid, and most major insurance plans. For those who don’t have insurance or who are underinsured, we offer sliding scale fees, Medicaid assistance, and prescription medication assistance program.
