Chronic Disease Management Services Offered at East Arkansas Family Health Center

At East Arkansas Family Health Center, we provide comprehensive chronic disease management and dedicated treatment of chronic medical conditions such as diabetes, high blood pressure (hypertension), obesity, and HIV/AIDS.

Our goal is to help you manage and improve your condition so you can feel your best and live a long, fulfilling life. This is why our caring team of providers emphasizes health education and prevention strategies. They will help you understand your chronic condition and determine how to best manage it through lifestyle changes, diet, exercise, and/or medications.

The self-management programs at EAFHC will help you learn everything you need to know to stay healthy and avoid medical complications from your long-term disease.

Our providers offer diabetes education to teach you how to check and manage your blood sugar, nutritional counseling to help you make healthy food choices, and assistance in developing an exercise regimen using simple, realistic goals tailored just for you.

In addition, we offer HIV/AIDS case management, education, and screening at the Arkansas Delta AIDS Care Center.
